PROGRAM REQUEST OF PARENTS/GUARDIANS Parents/Guardians are requested to call the Robbinsdale Area Schools Transportation department and request that the student be dropped off at Sparrow Institute on the days that they will attend the After-school program. The Transportation department can be contacted at 763-504-8107 or 763-504-8108. Please call prior to completing the registration, since the bus number and estimated arrival times are part of registration. Upon the completion of the registration, Sparrow Institute will contact the Transportation department to verify this information. SPARROW INSTITUTE PLEDGE TO STUDENTS Sparrow Institute will use its best efforts to assist the student in obtaining his/her maximum potential in the martial arts. We are committed to providing and maintaining professional facilities, staff and regular class schedule. The student, understanding that training is a service where two way communication is essential to the effective servicing of the student, agrees to communicate his/her needs and/or problems in a timely manner. TERMS AND CONDITIONS Lessons scheduled may be modified the the discretion of Sparrow Institute from time to time for holidays, competitions, and other school activities. Sparrow Institute shall give notice of such modifications by general announcements. Please note that if there is no school, there will NOT be an after-school class that day either. The parties agree that the costs for uniforms, equipment, materials, books, competition fees and other class/training related activities are not covered by this document and remain the responsibility of the student. The student also agrees that the names of students and any photographs or motion pictures taken during Sparrow Institute classes or activities may be used for promotional purposes. The student expressly accepts the risks of participation in martial arts and/or yoga, including the possibility of injury from physical exertion, and hereby releases and agrees to waive any claim of damages against Sparrow Institute and associated programs including, but not limited to, the executive board of Sparrow Institute, any members of Sparrow Institute boards or committees, instructors of any program/class, agents, employees, and property landlord (specifically Timberland Partners) for any injury or loss caused to the student while in, about, or as a result of being at Sparrow Institute, or resulting from activities at Sparrow Institute or related to the student's participation in activities, including related travel and competitions, or for any other damages from the student's participation in the martial arts. The student also agrees, that neither Sparrow Institute, nor any associated program will be responsible for any loss or injury to personal property and any person while in, about, or as a result of being at the school, and will not be responsible for any actions of other students or any other person. The student has the option to cancel this agreement within seventy-two (72) hours and obtain a full refund. There are no other agreements or promises other than as represented in this agreement.
